Licensed Occupational Therapist (OT) / Physical Therapist (PT)

Job Summary

Homeside Rehab is seeking compassionate and skilled Occupational Therapists (OTs) and Physical Therapists (PTs) to join our home health team in Hazleton, PA .

In this role, you will provide individualized, evidence-based therapy services to patients in the comfort of their homes. This is a great opportunity for therapists who value schedule flexibility, one-on-one patient care, and a supportive clinical and administrative team.

Job Responsibilities

• Evaluate patients and develop individualized treatment plans based on clinical assessments, functional needs, and patient goals

• Provide skilled Occupational Therapy or Physical Therapy interventions within your professional scope of practice

• Help patients improve mobility, strength, motor skills, functional independence, and overall quality of life

• Monitor patient progress and adjust treatment plans as clinically appropriate

• Educate patients and caregivers regarding treatment goals, home exercise programs, safety, and functional strategies

• Maintain accurate and timely clinical documentation in accordance with company and regulatory requirements

• Collaborate with the clinical team to ensure coordinated and effective patient care

• Communicate professionally with patients, caregivers, office staff, and other members of the care team

Job Qualifications

• Active Pennsylvania Occupational Therapist (OT) or Physical Therapist (PT) License – Required

• Basic computer proficiency for electronic documentation

• Strong clinical assessment and interpersonal communication skills

• Ability to work independently in a home health environment

• Valid driver's license and reliable transportation for home visits

• Pediatric experience preferred

• Minimum of 1 year of experience in Home Health, Outpatient, Rehabilitation, or Skilled Nursing Facility (SNF) settings preferred

Job Details

• Job Type: Full-Time, Part-Time, Per Diem

• Location: Hazleton, PA

• Pay: $65–$80 per hour

• Schedule: Flexible – choose your own availability

• Day and evening shifts available

• Monday through Friday

• Weekend availability as needed
