Your Role as a Bridge Care Coordinator Responsibilities Visiting patients in their homes and providing peace of mind. Educating families about available healthcare services. Connecting patients with community resources. Helping transition patients between care services, including hospice. What We're Looking For 18+ years old Healthcare Experience Required Strong organizational & computer skills Ability to work independently Valid driver's license & clean driving record Reliable Transportation & Current Auto Insurance (MVR Check Required) Negative TB test Ancora is a division of Aegis Healthcare.
