We are seeking an experienced Occupational Therapist (OT) Home Health for a 13-week contract serving Alameda County, California. The Occupational Therapist will provide skilled home-based therapy services while prioritizing Start of Care visits, supervising Occupational Therapy Assistants, and maintaining timely clinical documentation and communication.

The position follows a productivity-based home health model with an expected 30 points per week . The therapist will manage a community-based caseload and may be required to expand the assigned territory based on patient care needs.

• Perform comprehensive home health occupational therapy evaluations and prioritize Start of Care visits according to clinical and scheduling requirements.

• Develop and implement individualized treatment plans designed to improve patient safety, independence, functional abilities, and activities of daily living.

• Supervise Occupational Therapy Assistants and coordinate therapy services to support continuity and quality of patient care.

• Complete mandatory next-day Start of Care reporting and submit clinical notes and other required documentation within 48 hours of each visit.

• Maintain timely communication with supervisors, intake teams, and office staff while meeting the expected productivity standard of 30 points per week.

• Minimum 1 Year of Home Health Occupational Therapy Experience Required.

• Current, Active California Occupational Therapist License Required.

• Current BLS/BCLS Certification Required.

• Ability to Supervise Occupational Therapy Assistants and Independently Manage Home Health Start of Care, Evaluation, Recertification, Resumption of Care, Revisit, and Discharge Visits Required.

• Valid Driver's License and Reliable Transportation Required for Patient-to-Patient Travel; Ability to Work Monday Friday and Expand the Assigned Territory as Needed Required.
