Contract | 13-Week Assignment | Monday-Friday, 8:00 AM-5:00 PM | Onsite

Key Responsibilities

Provide physical therapy evaluation and treatment services within a skilled nursing facility (SNF) setting
Develop and implement individualized treatment plans for subacute and geriatric rehabilitation patients
Monitor and document patient progress, adjusting treatment plans as clinically appropriate
Collaborate with interdisciplinary care teams to support patient recovery and discharge planning
Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards

Required Qualifications

Active Physical Therapist (PT) license in the state of Florida
Graduate of an accredited Physical Therapy program
Ability to work in a skilled nursing facility environment
Strong clinical documentation and communication skills

Preferred Experience / Skills

Prior experience in a subacute, SNF, adult, or geriatric rehabilitation setting
Familiarity with SNF regulatory and compliance requirements
