Home Health Registered Nurse (RN) - Upper Marlboro, Maryland

Deliver high-quality, patient-centered nursing care as a Home Health Registered Nurse. This full-time, 13-week contract role is dedicated to serving patients in their homes, providing comprehensive assessments, developing individualized care plans, and fostering positive outcomes through expert clinical skills and compassionate support. Schedule Monday Friday, 8:00 AM 5:00 PM Responsibilities Conduct thorough OASIS assessments in patient homes and craft tailored care plans. Administer skilled nursing care, including wound management, medication administration, IV therapy, and chronic condition oversight. Monitor patient status, identify health changes, and communicate promptly with healthcare team members and physicians. Educate patients and families about conditions, medications, treatments, and preventive care. Ensure comprehensive, timely documentation via the Home Care Home Base (HCHB) platform. Work collaboratively with interdisciplinary team members including PT, OT, social workers, and health aides to ensure coordinated care. Attend required case conferences, staff meetings, and pursue continuing education. Comply with all regulatory and agency policies concerning patient care and documentation. Qualifications Active RN licensure, in good standing, for the state of practice. Minimum one year of recent field-based Home Health nursing experience. Proficient with HCHB and experienced with OASIS documentation. Strong clinical judgment and assessment capabilities. Reliable transportation, valid drivers license, and current auto insurance. Comfortable working independently and organizing a flexible field schedule. Excellent written, verbal, and organizational skills. Preferred Bachelor of Science in Nursing (BSN). Prior experience managing 2532 points per week. Familiarity with Medicare and home health regulatory requirements.
