Manager, Population Health - Corporate

Role Overview As a Manager, Population Health, you will lead the design, implementation, and refinement of population health programs that improve quality and reduce costs for Oak Street Health patients, providers, and the business. Core Responsibilities Support the execution and ongoing refinement of population health programs. Work closely with functional leaders (Product, Finance, Data Science) in the central office to assist with strategy and program design. Collaborate with regional leaders and Care Teams to ensure effective implementation of clinical and business priorities. Lead communication and content development for key population health metrics, including quality, utilization, and financial measures. Build relationships with Treehouse team members, clinicians, and external partners to drive innovation and improvement across centers. Perform other duties as assigned. Qualifications 5+ years of experience in consulting, healthcare, population health, or related fields working with healthcare providers. Ability to build trust and confidence across a range of stakeholders. Track record of effectively solving complex problems. Strong project management skills and ability to manage multiple priorities. Experience developing business cases, ROI analysis, and financial analysis. Proficiency in MS Office, Google Apps, and project management software. Strong written and oral communication skills. Compensation and Benefits Anticipated Weekly Hours: 40 hours per week. Pay Range: $66,330.00 - $145,860.00 (base salary).
