Key Responsibilities
- Deliver after?hours hospice nursing visits, urgent care interventions, and skilled assessments
- Manage triage calls, determine needed interventions, and complete in?person visits as required
- Provide pain and symptom management in accordance with hospice standards of care
- Educate patients and families on hospice philosophy, medications, and care expectations
- Document all assessments and interventions accurately and efficiently
- Communicate promptly with the interdisciplinary hospice team (IDT) to ensure continuity of care
- Participate in QAPI and quality improvement initiatives
- Must be able to work in both King and Pierce Counties
Qualifications
- Current Washington State Registered Nurse (RN) license
- Hospice agency experience required or the ability to train full day times until independent
- Current AHA BLS certification
- Strong clinical assessment and independent decision?making skills
- Ability to work autonomously during evening, night, and early?morning hours
- Reliable, insured vehicle and valid driver's license
