Team Nurse - Primary Care/JP- 32 hour/Week

$32.24 - $55.32 per hour

Reporting directly to the Practice Manager and the Senior Director of Nursing, the Team Nurse is responsible for performing the duties of an RN/LPN in an outpatient primary care or family medicine practice. The Team Nurse serves as a clinical resource for the medical practice and provides high‑quality, direct patient care in a team‑based model to our Beth Israel Deaconess HealthCare (BIDHC) patients. The Team Nurse enhances the safety, effectiveness, and efficiency of patient care by supporting and delegating clinical and administrative functions to Medical Assistant (MA) staff, educating and training the MA team and patients on preventive, chronic, and episodic health care issues. In collaboration with primary care providers, the Team Nurse coordinates patient‑centered care, supports transitions between levels of care, and counsels patients in improving their health status. Primary Responsibilities Coordinate and facilitate quality clinical management of a broad outpatient population across the continuum of care. Collaborate with providers to identify needs for preventive care, evidence‑based chronic disease management, and care coordination; provide general and specific health education; work directly with patients to identify and monitor self‑management goals. (essential) Collaborate on care with community‑based providers, follow up on ED visits and inpatient discharges, and assist providers with exams, treatments, and procedures as needed. (essential) Provide skilled telephonic triage and assessment, education, collaboration, and coordination of healthcare and community resources. Conduct direct patient care visits, including Annual Wellness Visits (AWVs) and Chronic Care Management (CCM) of BIDHC patients. (essential) Responsible for INR management and teaching for patients on Coumadin using a standardized protocol. (essential) Administer vaccines and medications by parenteral, oral, rectal, or nasal route per provider orders and BIDHC Medication Administration Policy. Provide patient education appropriate for the administered medication. (essential) Perform a wide range of nursing activities as needed under the provider’s direction, such as wound care and point‑of‑care testing; accurately document all aspects of patient care in the Electronic Health Record; schedule tests and referrals for specialty care on provider orders. (essential) Monitor and ensure follow‑up of all abnormal test and imaging results under the provider’s direction. (essential) Oversee inventory and ordering management of clinical supplies and equipment as necessary; ensure safe, accurate, and clinically appropriate collection, storage, and processing of all biologic specimens. (essential) Provide oversight, delegation, and act as a resource to unlicensed staff, including MAs, Certified Nursing Assistants, and Nursing Students; provide ongoing education and training to practice staff; maintain compliance with mandatory annual training. (essential) Ensure that PHI is used only for patient treatment, payment, and operations; uphold patient safety practices to prevent or eliminate risk of patient harm; promptly report adverse patient events and near misses to the BIDHC Risk Management Department. (essential) Required Qualifications Graduate from an accredited Nursing Program; Bachelor’s degree preferred. Registered Nurse license required, or Licensed Practical Nurse registration required; Basic Life Support certificate required. 1–3 years of related work experience required. CPR certification. Strong interpersonal skills; strong patient communication and education skills. Advanced skills with Microsoft applications (Outlook, Word, Excel, PowerPoint, Access) and other web‑based applications; ability to produce complex documents, perform analysis, and maintain databases. Competencies Decision Making: Make decisions guided by instructions and practices requiring some interpretation; recommend solutions for moderate‑complexity problems. Problem Solving: Address varied problems using observation, knowledge, and skills; interpret situations based on precedents. Independence of Action: Follow precedents and procedures, set priorities, and organize work within general guidelines; seek assistance when needed; work progress monitored by supervisor. Written Communications: Communicate clearly and effectively in written English with internal and external customers. Oral Communications: Comprehend and converse in English to communicate effectively with staff, patients, families, and external customers. Knowledge: Demonstrate full working knowledge of standard concepts, practices, procedures, and policies; apply them in varied situations. Team Work: Work collaboratively in small teams to improve operations through idea generation, issue identification, and respect for team members. Customer Service: Provide high‑level customer service to patients, visitors, staff, and external customers in a professional, respectful manner; remain calm in stressful situations. Age‑Based Competencies Young adult: 16–30 years. Middle age: 31–60 years. Elderly: 61 years and older. Social / Environmental Requirements Work requires periods of close attention and concentration for up to four hours without interruption. Work requires constant response to changing circumstances and the use of new information to adjust approaches. Potential exposure to adverse environmental conditions, including protective equipment (respirator, earplugs, mask, gloves, eyewear), infectious diseases, airborne pathogens, and bloodborne pathogens. Health Care Worker status: regular, day‑to‑day contact with patients and staff; may involve close contact within three feet for at least five minutes. Physical Requirements The role is physically demanding, requiring frequent standing, walking, and endurance for 3–4 hours without a break. Employees must be able to lift up to 50 pounds occasionally and up to 20 pounds frequently. Occasionally, employees may need to push a wheelchair or wheeled bed with a patient weighing up to 250 pounds (with assistance) or reposition a patient weighing up to 100 pounds without assistance. The position involves frequent use of keyboards, bending neck and waist, palm grasping, fine manipulation, and reaching above or below shoulder height. Pay Range $32.24 – $55.32 per hour. The base hourly wage range may vary based on seniority, education, training, experience, certifications, geography, and other permissible factors. Compensation may exceed the base rate depending on shift differentials, call pay, premium pay, overtime pay, and other applicable practices.
