Home Care Intake Coordinator

Addus Family Company is seeking a Weekend Intake Coordinator to support our mission of helping seniors and vulnerable adults remain safely at home. In this role, you will be the first point of contact for weekend referrals and service requests, gathering clinical and personal information, verifying eligibility, and coordinating timely starts of care. You will collaborate closely with branch, scheduling, and clinical teams, document accurately in our systems, and ensure compliance with regulations and payer requirements while providing compassionate, professional service to families and partners.
