Req Number MED-26-00005 Location IBHS Burlington - Riverbank (Burlington, NJ) Full-Time/Part-Time Full-Time Number of Openings 1 Category Medical / Nursing Job Summary The Registered Nurse is responsible for the coordination of health services, conducting and/or supervising nursing assessments and plans of care. Will collaborate with a psychiatrist and all team members to develop rehabilitation and recovery plans. Duties and Responsibilities Conduct quarterly nursing assessments and develop plans of care for all consumers in the program. Assign duties to LPN and CNA staff in accordance with the assessments and plans. Plan and conduct and/or supervise the provision of health education classes on nutrition, exercise, disease management, medications, hygiene, smoking cessation and other health‑promoting classes. Coordinate with primary care providers and specialists, ensuring preventive as well as specialized medical services are provided on a regular basis. Assign duties to LPNs and CNAs to assist consumers with appointments and follow‑up care. Collaborate with the Housing Support Specialist, psychiatrist and the consumer to actively participate in the rehabilitation and recovery plan and assist with activities that enable consumers to reach their goals. Identify assistive devices and/or specialized medical equipment needed for individual consumers; work in collaboration with the medical provider and the Housing Support Specialist to ensure such equipment is procured. Direct consumer medical care and act as the ongoing point of contact for medical providers, community hospitals, psychiatrist and any other specialists involved in the consumers’ care. Train staff and consumers on the importance of the mind‑body connection, how it affects each other, and how to treat both simultaneously. Receive and review referrals for consumer evaluation and coordinate admission of consumers with members of the MESH team. Conduct periodic chart audits. Check nursing team member documentation for accuracy and completeness prior to signing. Assist the Program Director in the formulation, revision, implementation, and evaluation of policies and procedures. With approval of the Program Director, develop a training schedule for nursing staff. Be responsible for nursing staff schedules. With the Program Director, conduct performance evaluations and initiate disciplinary actions, as necessary. Position Requirements A valid NJ registered nursing license and a valid driver’s license. At least one year of experience in mental health preferred. Ability to work flexible hours and to be on‑call for emergencies and consultation.
