For 50 years, VNA of Florida has helped patients receive high-quality care in the comfort of home. We are seeking an experienced Intake RN to support our clinical operations team by reviewing referrals, coordinating admissions, and ensuring patients receive timely access to care.

What You'll Do

• Review incoming home health referrals for clinical appropriateness
• Communicate with hospitals, physicians, case managers, and referral partners
• Verify eligibility and services ordered
• Coordinate admissions and start-of-care scheduling
• Support clinicians with referral and intake questions
• Ensure compliance with Medicare and agency guidelines
• Assist with care coordination and patient transitions

What You'll Need

• Active Florida RN License
• Home health experience strongly preferred
• Knowledge of Medicare home health regulations
• Strong organizational and communication skills
• Experience with EMR systems (HCHB preferred)
• Ability to prioritize and manage multiple referrals
