We are seeking a skilled and patient-focused LVN (IV Certified) to join our growing home health team. Position Overview As an LVN, you will provide skilled nursing care in a home health setting, working under the supervision of a Registered Nurse or physician. This role requires strong clinical skills, independence, and a compassionate approach to patient care. Key Responsibilities Perform comprehensive patient assessments and monitor ongoing progress. Develop, implement, and follow individualized care plans. Administer medications, including injections and IV therapy. Provide wound care, including dressing changes and monitoring for infections. Educate patients and their families about their conditions, medications, and care plans. Accurately document patient care, assessments, and progress notes. Observe and report changes in patient condition to supervising clinicians. Collaborate with interdisciplinary teams to ensure quality care. Qualifications Active LVN license (California). IV Certification (required). Valid BLS Certification. Minimum 1 year of nursing experience. Home Health or Hospice experience preferred. Strong skills in medication administration, wound care, and patient assessment. Excellent communication and interpersonal skills. Ability to work independently in a home health environment. Ability to lift up to 50 lbs as needed. Valid Driver’s License and reliable transportation. What We Offer Competitive salary starting at $74880 to 79040 annually.
