They report directly to the Medical Director regarding clinical issues and activities related to the delivery of medical care. APRN are expected to be familiar with the philosophy, goals and objectives of the Center and work collaboratively to achieve them. They must be cognizant of, and comply with, all Center policies and procedures, as well as pertinent state and federal regulations. APRN provides the full scope of primary care services, which fall under his/her field of training, including but not limited to diagnosis, treatment, coordination of care, preventive care and health maintenance of their patients.Essential Job FunctionsReasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.Obtain health history and perform physical examinations.Diagnose and direct counseling for each patient on a plan for treatment.Prescribe medication in accordance with state statute and professional practice guidelines.Maintains a problem-oriented health record for each patient seen in accordance with the Centers established Health Information Management policies and procedures.Assures continuity of care by providing comprehensive medical services on a timely basis as per Clinic schedule within the Centers scope of practice and the providers training.Provides excellent internal/external customer service via telephone, videoconference, chat, or face-to-face contact to assist patients with their health care needs.Perform minor surgical procedures.Arrange referral for patients requiring services not offered at the Medical Center.Review incoming reports (e.g. lab, x-ray, EKG) sign, date and follow-up in a timely manner.Follows established departmental and medical center policies, procedures, and objectives; continuous quality improvements objectives and safety, environmental, and/or infection control standards.Participates in specified health promotion, education and/or prevention programs as needed i.e. diabetes collaborative/health fairs etc.; attends and participates in medical center meetings/departmental meetings and other medical center functions as required by the Center.Assist in updating protocols and principles of practice as requested.Assist in outside medical public relations functions. Trains, mentors and supervises students and clinical support staff, as appropriate Supervises mid-level practitioners (physician assistants and nurse practitioners).Perform other duties as assigned by the supervisor.QualificationsSupervisory ResponsibilitiesThis position has no supervisory responsibilities.Required EducationMasters Degree from accredited Nursing Program.Required Experience3 + years in Primary Care.Required Licenses and CertificationsValid State Nurse Practitioner License.Current State D.E.A. Certificate.Certification by one of the Nurse Practitioner certifying bodies (ANCC or AANP) as a Family Nurse Practitioner (FNP) or Adult Gerontology NP.Current BLS and ACLS certification.Required Knowledge, Skills, and AbilitiesAny combination of education, training, and experience which demonstrates the ability to perform the duties and responsibilities as described including related work experience.Preferred QualificationsTwo years of experience working with a multicultural patient population delivering patient-centric programs that are high quality and cost effective, preferred.Financial ResponsibilitiesThis position does not currently have financial responsibilities.Budget ResponsibilitiesThis position does not currently have budget responsibilities.LanguagesAdvanced English is required.Spanish and Creole are preferred.TravelRequired: Flexible to travel per business needs.Physical DemandsThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.Includes all patient-facing clinical staff ( Medical Assistants, Phlebotomists, Mammography Technologists, Radiologic Technologists, Multi-Specialty staff, Providers) as well as Facilities Technicians. These positions typically require prolonged standing and walking, frequent bending, reaching, lifting, pushing, and pulling of equipment or supplies (up to 50 pounds), manual dexterity, and compliance with infection control and safety protocols.
