Harmony Medical Clinic is seeking a compassionate and engaging Registered Nurse (RN) or Licensed Practical Nurse (LPN) to serve as a Digital Health Nurse Educator for our Telehealth Initiative.

The Digital Health Educator will help patients and community members understand, access, and confidently use telehealth and digital healthcare resources. This position combines the clinical knowledge of a licensed nurse with patient education, technology support, and community outreach.

The ideal candidate enjoys educating others and is comfortable working with older adults, rural communities, and individuals who may have limited experience with technology or virtual healthcare.

Key Responsibilities

• Conduct outreach to community organizations, churches, senior organizations, residential and community facilities, and other community partners to identify opportunities for telehealth and digital health education and engagement.
• Develop and maintain relationships with community partners to coordinate educational sessions, presentations, demonstrations, and outreach activities.
• Identify opportunities to reach individuals who may experience barriers to accessing traditional healthcare, particularly within rural and underserved communities.
• Coordinate and participate in community-based telehealth education events and outreach activities.
• Educate patients and community members about telehealth, virtual healthcare, and the appropriate use of digital health resources.
• Provide individual and group telehealth education in clinic and community settings.
• Provide hands-on education using smartphones and tablets, including helping individuals understand how to access, prepare for, and participate in telehealth appointments.
• Provide hands-on education regarding remote patient monitoring devices and help individuals understand how these technologies are used to support ongoing healthcare monitoring.
• Demonstrate the basic use of approved telehealth and remote monitoring technology in an easy-to-understand, patient-friendly manner.
• Help identify barriers to telehealth adoption, including limited technology experience, connectivity concerns, and digital health literacy.
• Provide health education within the appropriate scope of RN or LPN practice.
• Assist with developing and distributing patient and community telehealth educational materials.
• Maintain documentation of outreach contacts, community partnerships, educational sessions, participant engagement, and other required initiative activities.
• Assist with collecting information needed to measure telehealth education, outreach, participation, and program outcomes.
• Follow up with community organizations and participants as appropriate to support continued engagement.
• Collaborate with Harmony Medical Clinic providers, staff, and initiative leadership to support the goals of the Telehealth Initiative.
• Maintain patient confidentiality and comply with HIPAA and applicable professional nursing standards.
• Complete other contracted telehealth education and outreach activities within the scope of the initiative.
