Social Worker - Case Manager Full Time / Day Shift

We are seeking a full-time Social Worker - Case Manager to join an interdisciplinary team, focusing on assessment, care coordination, and discharge planning for patients across all age groups. Key Responsibilities Include Assessing patients' discharge needs within 24 hours of admission and throughout the stay Participating in multidisciplinary meetings to address discharge barriers Educating patients and families on discharge plans and post-acute care Evaluating social determinants of health and risk factors impacting care Securing authorization for post-acute services and documenting all interactions Supporting ethical practices, patient rights, and compliance with regulations Providing psychosocial assessments and interventions for high-risk populations Connecting patients with community resources and ensuring safe transitions of care Mentoring and guiding unlicensed team members as needed Qualifications Master's degree in Social Work and LICSW or LCSW licensure At least three years of social work experience, preferably in acute care Current BLS certification Preferred: case management certification and experience in hospital settings This role emphasizes patient advocacy, risk assessment, and effective discharge planning to improve patient outcomes and reduce readmissions. #J-18808-Ljbffr Northshore Home Health Care
