Hospice RN Case Manager: Coordinate Compassionate Care
Community Healthcare of Texas
Community Healthcare of Texas is seeking an RN Case Manager to coordinate hospice services, meet with patients and families, and manage the ongoing plan of care. You will administer medications, supervise LVN/CNA staff, participate in interdisciplinary group meetings, and handle on-call rotations to admit new patients. Texas RN license and hospice experience preferred; valid driver’s license required.
