Rheumatology Nurse Navigator & Care Coordinator Lead

The University of Miami Health System seeks a Full-Time Rheumatology Nurse Navigator 1 to provide proactive clinical navigation for new and complex rheumatology patients throughout the care continuum. You will perform pre-visit chart preparation, identify barriers to care, educate patients, and coordinate cross-disciplinary care with APRN, physicians, nurses, CBOR staff, pharmacies, infusion services, and external partners.
