Pay: $23.00 per hour-$25.00 per hour

Job Summary:  The Home Care Intake & Referral Coordinator serves as the primary link between the Licensed Home Care Services Agency (LHCSA) and Certified Home Health Agency (CHHA) operations to ensure the seamless transition of patients across the continuum of care. This role is responsible for identifying and coordinating appropriate referrals from the LHCSA to the CHHA, monitoring referral outcomes, preventing lost opportunities, and ensuring patients receive the most appropriate level of care based on their clinical needs.

The Home Care Referral Liaison works collaboratively with Intake, Clinical Operations, Care Coordination, Nursing, and Business Development teams to maximize patient retention, support census growth, and ensure compliance with applicable regulations and payer requirements. The individual will also utilize ePACES and other systems to verify eligibility and support referral processing activities.

Job Responsibilities:

Referral Management & Business Retention

• Serve as the primary liaison between LHCSA and CHHA operations.

• Coordinate referrals from the LHCSA to the CHHA to ensure timely evaluation and admission.

• Track and monitor referral conversion rates and identify barriers to admission.

• Investigate missed referral opportunities and recommend corrective actions.

• Develop and maintain reports related to referral activity, admissions, denials, and lost business opportunities.

• Partner with Intake and Clinical leadership to ensure referrals are processed timely and efficiently.

Care Coordination

• Communicate with LHCSA coordinators, nurses, administrators, and CHHA intake personnel regarding referral status.

• Facilitate timely exchange of patient information and clinical documentation.

• Coordinate smooth transitions of care between service lines.

• Escalate referral delays, authorization concerns, staffing challenges, or other barriers to appropriate leadership.

Eligibility & Payer Verification

• Utilize ePACES to verify Medicaid eligibility and managed care enrollment.

• Review payer information and identify coverage requirements impacting admissions.

• Assist with obtaining authorizations or supporting documentation as needed.

• Ensure referrals meet payer, regulatory, and agency admission criteria.

Operational Performance

• Monitor referral workflow from identification through admission decision.

• Maintain accurate referral tracking databases and reporting tools.

• Analyze trends impacting admissions, referral leakage, and conversion performance.

• Participate in quality improvement initiatives related to referral management and census growth.

• Provide recommendations to leadership regarding process improvements.

Relationship Management

Build strong working relationships with LHCSA and CHHA operational teams.

Serve as a resource to field staff and coordinators regarding available services and referral processes.

Promote collaboration between agencies to improve patient outcomes and organizational performance.

Compliance

Ensure all referrals are handled in accordance with NYSDOH regulations, Medicare Conditions of Participation, Medicaid requirements, and agency policies.

Maintain confidentiality and HIPAA compliance at all times.

Other Duties

Perform other duties as assigned.

Qualifications:

Required

• Minimum of 3 years of home care experience.

• Experience working within both a:

  o Licensed Home Care Services Agency (LHCSA)

  o Certified Home Health Agency (CHHA)

• Working knowledge of Medicare, Medicaid, Managed Care, and Home Health regulations.

• Experience utilizing ePACES for Medicaid eligibility verification.

• Strong understanding of CHHA admission criteria and referral processes.

• Experience with home care EMR systems.

• Strong analytical and problem-solving skills.

• Excellent verbal and written communication skills.

• Ability to manage multiple priorities and deadlines.

  Preferred

• Intake, care coordination, or referral management experience.

• Experience analyzing referral trends and operational performance metrics.

• Knowledge of New York home care regulations and reimbursement models.
