Behavioral Health Case ManagerHarbor is seeking a Behavioral Health Case Manager to join the Toledo team! This position provides specific, measurable, and individualized community-based case management services to adults experiencing mental health and behavioral health challenges. Services focus on helping individuals succeed in the community, identify and access needed resources, improve daily functioning, increase independence, and enhance community integration to support an improved quality of life.Position is full time, 40 hours per week.RequirementsEducation:Must have either:Bachelor's degree in counseling, social work, psychology, or a closely related behavioral/mental health field from an accredited college or university.High School Diploma or GED and 3 years of experience in a behavioral health or related field may be accepted in lieu of Bachelor's degree.Experience:Three years of experience in a behavioral health or related field required if highest education level is High School Diploma/GED.Proficiency in computer use, including MS Office 365 (Outlook, Word, Excel), electronic health records, and virtual meeting platforms.Licensure and Certifications:Meets requirements to be a Qualified Mental Health Specialist (QMHS) and successfully completes required training before services are rendered. Training includes competencies including, at a minimum, an understanding of mental illness or substance use disorder treatment and recovery, psychiatric and substance use disorder symptoms and their impact on functioning and behavior; how to therapeutically engage a person with mental illness or a person in substance use disorder treatment and recovery; crisis response procedures; an understanding of the community behavioral health system, social service systems, criminal justice system and other healthcare systems; de-escalation techniques and understanding how your own behavior can impact the behavior of others.Must successfully demonstrate the basic competencies listed above, as determined by your supervisor's review, within 90 days of hire.Complete 20 hours of CEUs every 24 months to meet non-licensed staff renewal requirements per the Department of Behavioral Health.CPR/First Aid and NCI/CPI Certifications required within 90 days of employment.Must have a valid driver's license, acceptable driving record, and show proof of personal auto insurance policy to be deemed insurable with Harbor's vehicle insurance carrier.For specialized programs:Youth Providers: CANS certification may be required within 90 days of employment.Substance Use Disorder Program: may require completion of license as a Chemical Dependency Counselor Assistant in Ohio.Preferred Qualifications:Experience providing case management to individuals representing multiple lived experiences (e.g., severe and persistent mental health diagnoses, substance use disorder clients, youth and adults) in a community mental health center setting.ResponsibilitiesEssential Functions and Primary Responsibilities:Serves as the primary clinician for assigned clients as evidenced by coordinating care across internal and external providers and ensuring updates to the Individualized Service Plan (ISP) are completed regularly and as needed.Responsible for completing documentation accurately, timely and in support of medical necessity, including but not limited to: progress notes, ISPs, needs assessments, discharge summaries, and other forms as required.Meets or exceeds billable direct service expectations established by Harbor.Provides individual and group community psychiatric supportive treatment (CPST) and therapeutic behavioral services (TBS), which could include facilitation of Day Treatment groups, according to accreditation and certification standards including:Coordination of individualized treatment planning with client, family/supports, and other providers.Identification of strategies or treatment options.Developing and providing solution focused interventions and emotional and behavioral management drawn from evidence-based psychotherapeutic treatments.Restoration of social skills.Restoration of daily functioning.Crisis prevention and amelioration.Collaboration with internal and external providers to assist clients in gaining access to essential community resources and increasing social supports and networks.Links clients to appropriate resources, entitlement programs, and provides advocacy for clients. Follows up with referral sources to provide relevant updates and information, as appropriate and agreed to by the client.When providing telehealth services to clients in their homes, complies with Harbor's telehealth policies, procedures, and utilizes Harbor authorized or provided telehealth equipment and software platforms.Transports clients as needed for appointments as determined by job assignment and in accordance with driving policies.Provides timely response to all client and agency communications via email, phone, or other department identified modes of communication (e.g., Teams).Completes all agency assigned trainings in a timely manner.May float to other locations to accommodate client or agency need.Keeps current with trends and developments related to essential job competencies including evidence-based practices for day treatment curricula and other services.Knowledgeable of mental health and substance use disorder symptoms and treatment to assist clients with co-occurring disorders.Understands and maintains professionalism and confidentiality per HIPAA, 42 CFR Part 2, and applicable state and local laws.Participates in meetings, committees, and agency initiatives as appropriate and approved by supervisor.General Competencies/Performance Expectations:Utilizes information technology systems and software applications effectively and in accordance with organizational standards and security protocols.Demonstrates sound judgment by analyzing information thoroughly, reaching timely conclusions, and taking appropriate, decisive action.Plans, prioritizes, and organizes work to ensure maximum efficiency, productivity, and quality outcomes.Actively seeks and utilizes supervision and feedback appropriately to support professional growth and effective job performance.Manages time and utilizes leave in accordance with organizational policies and procedures.Performs additional duties and responsibilities as assigned to support departmental and organizational needs.Working Conditions:Community/field-based position including provision of services in clients' homes, agency offices, and other community settings. Some services may be provided from the office virtually to clients in their homes.Travel expectations include driving an agency vehicle while transporting clients or driving their own vehicle for work when not transporting clients.Exposure to confidential information.Potential exposure to behavioral, crisis, or environmental situations.This position is union.
