Nuestro programa de atención primaria multidisciplinaria está diseñado para abordar las necesidades físicas, conductuales, sociales y culturales de los ancianos hispanos elegibles para Medicare. Responsabilidades Interviene con pacientes y familias sobre las consecuencias emocionales, sociales y financieras de la enfermedad y/o discapacidad. Realiza visitas en persona a los hogares de los pacientes, accede y moviliza recursos familiares/comunitarios, proporciona educación al paciente sobre el manejo de enfermedades agudas y crónicas, colabora con el equipo de atención multidisciplinaria, mantiene conocimiento sobre Medicare, Medicaid y otros beneficios del programa, asiste con la coordinación del cuidado y participa en las operaciones diarias de una práctica de atención primaria. Requisitos La oferta de trabajo es para un Trabajador de Salud Comunitaria en Suvida Healthcare en Houston, TX, EE. UU. El papel requiere una comunicación terapéutica excelente con pacientes, familias y cuidadores, así como articular la misión de la empresa en relación con la satisfacción y los resultados de los pacientes. El candidato ideal debe ser compasivo, amable, de mente abierta, experimentado en trabajo en equipo y capaz de comunicarse eficazmente entre diferentes culturas, habilidades, géneros, etnias y razas. Se prefiere competencia bilingüe en inglés y español.
