As a Care Coordinator, you'll help patients overcome these barriers while working alongside an incredible interdisciplinary team that is committed to whole-person care.This is more than a care coordination position—it's an opportunity to build meaningful relationships, advocate for patients, strengthen community partnerships, and make a lasting impact every day.What You'll Do As our Care Coordinator, you'll be an essential member of our integrated healthcare team by:Coordinating care for patients with complex medical, behavioral health, dental, and social needs.Helping patients navigate healthcare services, insurance, financial assistance, and community resources.Connecting patients with housing, transportation, food assistance, medication programs, and other community supports.Identifying barriers to care and developing solutions that improve patient outcomes.Collaborating with Primary Care, Behavioral Health, Dental, and community partners to ensure coordinated, patient-centered care.Providing ongoing patient outreach and follow-up to encourage engagement and successful treatment.Advocating for patients while empowering them to become active participants in their own healthcare.Building and maintaining relationships with community organizations to expand resources available to our patients.Participating in quality improvement initiatives that enhance patient care and clinic operations.Supporting operational continuity by providing cross-coverage for the Referral Coordinator and assisting the Behavioral Health Consultant with non-clinical patient navigation and resource coordination.What We're Looking For We're seeking someone who is:Passionate about helping others.A natural relationship builder and collaborator.Organized, adaptable, and able to manage multiple priorities.Skilled at problem-solving and thinking creatively.Compassionate while maintaining professional boundaries.Comfortable working independently and as part of a multidisciplinary healthcare team.QualificationsBachelor's degree in Human Services, Social Work, Psychology, Public Health, Healthcare Administration, or a related field required.Experience in healthcare, case management, care coordination, behavioral health, or community services preferred.Knowledge of community resources and social drivers of health preferred.Why Bridge Community Health Clinic?
