Case Manager for Veterans - LA/OC

Job Summary

A mission-driven nonprofit organization is seeking a Case Manager for Veterans - LA/OC to support the Veteran Directed Care Program across Los Angeles and Orange Counties. This direct hire opportunity is ideal for a compassionate, organized professional who wants to help Veterans remain independent in their homes while accessing the care, services, and resources they need.

In this field-based and hybrid role, you will work closely with Veterans, care employees, fiscal management partners, and local program stakeholders to develop personalized support plans, manage self-directed budgets, and coordinate essential services. Key Responsibilities

- Maintain an active caseload of at least 50 Veterans through the Veteran Directed Care Program.
- Conduct activities of daily living assessments, initial intakes, and quarterly in-person home visits.
- Develop, document, and implement Veteran-directed spending plans and individualized support plans.
- Assist Veterans with self-directed budget management, service selection, and independent living purchases.
- Guide Veterans through hiring, training, supervising, and terminating care employees, including required paperwork.
- Coordinate with fiscal management services, VA Medical Center contacts, and community resources while maintaining accurate HIPAA-compliant documentation.

Compensation and Benefits

- Pay rate: $24.00 to $25.00 per hour.
- Position type: Direct hire, full-time, 40 hours per week, non-exempt.
- Location: La Habra, CA, with service coverage across Orange County and Los Angeles County.
- Work arrangement: Field-based and hybrid.
- Travel requirement: Up to 60% local and regional travel for home visits and direct field services.

Required Qualifications and Skills

- Manage a caseload of Veterans receiving independent living and in-home support services.
- Complete assessments, care planning, documentation, and ongoing service coordination.
- Support Veteran-directed budgeting, employee paperwork, timesheet review, and spending plan compliance.
- Conduct required field visits throughout Los Angeles and Orange Counties.

- Bachelor’s degree from an accredited college or university.
- At least 2 years of experience working with individuals with disabilities, older adults, and/or Veterans, or an equivalent combination of education and experience.
- Valid California Driver’s License, reliable transportation, and current auto insurance.
- Working knowledge of Independent Living philosophy, the Americans with Disabilities Act, the Rehabilitation Act, and the Fair Housing Act.
- Proficiency with MS Windows, Microsoft Office, Google Suite, and database management.
- Strong organizational skills with the ability to manage budgets, deadlines, documentation, and independent field work.
- Excellent communication skills with a professional, respectful, and proactive approach to clients, team members, and community partners.
- High level of ethics, accountability, adaptability, and professionalism.
- Ability to travel frequently, enter and exit a vehicle regularly, work at a desk or while driving, and occasionally lift or move up to 10 pounds.

Preferred Qualifications

- Bilingual English and Spanish communication skills.
- Experience with Veteran services, case management, home and community-based services, or self-directed care programs.
- Familiarity with VA-related programs, fiscal management services, or nonprofit human services environments.
