Position Overview

The LPN Unit Care Coordinator plays a vital role in overseeing, implementing, coordinating, and managing patient care. This position involves direct interaction with patients, their families, nursing personnel, and other stakeholders within the unit, ensuring compliance with all relevant laws, regulations, and organizational standards.

• Completion of an accredited LPN program.
• Current licensure as a Licensed Practical/Vocational Nurse (LPN/LVN) in the applicable state, with a requirement to maintain an active license in good standing throughout employment.
• Preferred: At least one year of experience in geriatric nursing.
• CPR certification is required upon hire or must be obtained during orientation; this certification must remain valid throughout employment.

• Possess advanced knowledge in the nursing field.
• Ability to make independent decisions when necessary.
• Familiarity with practices, procedures, and the legal framework governing operations in a post-acute care setting.
• Implement and interpret departmental programs, goals, objectives, policies, and procedures.
• Demonstrate proficiency in all competency areas, including patient rights, safety, and sanitation.
• Maintain professional relationships with colleagues, vendors, and other stakeholders.
• Ensure confidentiality of all proprietary and sensitive information.
• Adhere to organizational policies, including those related to harassment and compliance.
• Exhibit integrity and professionalism by following the organization’s Code of Conduct and completing all required compliance training.

• Effectively oversee the daily operations of unit nurses and CNAs, providing leadership on the floor.
• Document patient information accurately and in a timely manner.
• Manage and coordinate patient care services through interpersonal interactions, helping patients achieve or maintain optimal physical, mental, and psychosocial well-being.
• Prepare and administer medications as prescribed by a physician with accuracy.
• Respond to emergency situations with a leadership approach, prioritizing patient and staff safety.
• Collaborate with RNs to coordinate patient care plans and services.
• Provide excellent customer service and maintain a positive attitude towards patients.
• Assist in the safe evacuation of patients when necessary.
• Demonstrate reliable and consistent attendance.
• Utilize reasoning skills and sound judgment in various situations.
• Communicate effectively and work collaboratively within an interdisciplinary team.
• Engage in physical activities such as sitting, standing, bending, lifting, pushing, pulling, stooping, walking, reaching, and moving intermittently throughout the workday.
• Read, write, speak, and comprehend the English language proficiently.
