Your Responsibilities

Provide direct nursing care in a home environment Assist with daily care needs and monitor patient condition Communicate effectively with care teams and families Ensure a safe, comfortable, and supportive setting for clients

Requirements

At least 1 year of recent LPN experience Active Connecticut LPN license Current BLS certification Authorized to work in the United States Reliable transportation and valid driver's license preferred Physical exam within the past year PPD or QuantiFERON within the past year, or chest X-ray within five years if previously positive
