Key Responsibilities\n

  • Conduct ongoing assessments to identify the unique emotional and psychological needs of patients.

  • Facilitate clear and effective communication with supervisors, direct care staff, families, and other stakeholders to track patient progress.

  • Prepare and maintain a schedule outlining appointments, home visits, and observations to monitor treatment effectiveness.

  • Coordinate home visits and outreach efforts, ensuring comprehensive care through in-home observations, chart reviews, and staff interviews.

  • Implement and monitor treatment plans, adjusting strategies as needed based on patient progress and feedback.
  • \n
  • Participate in Interdisciplinary Team Meetings to collaboratively address patient care and program goals.
  • \n
  • Document patient progress through monthly and quarterly notes, ensuring compliance with regulatory standards.
  • \n
  • Provide staff training to ensure competency in program procedures and documentation requirements.
  • \n
  • Conduct follow-up interviews, home visits, and observations to ensure continuous support for patient needs.

  • Act as a liaison with public and mental health organizations to foster community support and resource sharing.

  • Assist in creating and refining training materials and procedures to enhance program efficacy for all participants.

Patient Population / Client Focus\n

The ideal candidate will work with diverse patient populations, including both adults and youth facing various mental health challenges, such as anxiety, depression, trauma, and substance misuse. Understanding and addressing the nuanced experiences of individuals from different backgrounds is vital for fostering an inclusive therapeutic environment.

Qualifications\n

  • A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.

  • An active and unrestricted professional counseling license (LPC) in the state of Tennessee.
  • \n
  • At least 2 years of relevant counseling experience, preferably in a clinical or outpatient setting.

  • A strong foundation in legal and ethical standards governing mental health practices.
  • \n
  • Exceptional interpersonal and communication skills, with the ability to establish rapport with a wide range of clients.

  • Demonstrated ability to function effectively within a healthcare team.

  • Familiarity with EMR software and digital documentation processes.

Preferred Experience\n

  • Specialization in family therapy, couples therapy, trauma-informed care, or substance misuse treatment is highly preferred.

  • Experience in the implementation of evidence-based practices and diverse therapeutic modalities.

Compensation and Benefits\n
