RN Case Manager- Heart of Hospice

Registered Nurse Case Manager – Heart of Hospice We offer a $5,000 sign‑on bonus for full‑time external candidates. This role is based at Heart of Hospice, part of LHC Group. Primary Responsibilities Make the initial nursing evaluation to determine eligibility for hospice services during a visit within forty‑eight (48) hours of referral. Identify the patient’s or family’s physical, psychosocial, and environmental needs and re‑assess as needed, no less than every fifteen (15) days. Initiate and coordinate the plan of care. Document problems, appropriate goals, interventions, and patient/family response to hospice care. Collaborate with the patient, family, attending physician, and other members of the IDG in providing patient and family care daily. Instruct and supervise the patient or family in self‑care techniques when appropriate. Maintain accurate and relevant clinical notes regarding the patient’s condition. Required Qualifications Current and unrestricted RN licensure in the state of practice. 1+ years of clinical experience. Current CPR certification. Current driver’s license, valid vehicle insurance, and access to a dependable vehicle or public transportation. Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client. State Specific Requirements (L.A.) For Louisiana: 2+ years of full‑time experience working as an RN. Two years of full‑time hospice experience as a licensed practical nurse may be substituted for the required RN experience. Preferred Qualifications Ability to work independently. Good communication, writing, and organizational skills. Benefits Salary range: $75,620 to $113,430 annually for full‑time employment.
