Position: Home Health Occupational Therapist

Location: Medford, OR and Surrounding Areas

Shift: Day 5x8-Hour (08:00 AM - 04:30 PM)

Job Description

We are seeking a dedicated Home Health Occupational Therapist to provide high-quality care in the home health setting. This position offers an opportunity to make a significant impact on the lives of patients by assisting them in improving their daily functioning and overall well-being.

Key Responsibilities

• Provide occupational therapy services to patients in their homes.
• Assess patients' needs and develop individualized treatment plans.
• Implement therapeutic interventions to help patients achieve their goals.
• Document patient progress and adjust treatments as necessary.
• Collaborate with other healthcare professionals to ensure comprehensive care.
• Maintain compliance with all regulatory, accreditation, and agency guidelines.

Qualifications

• Minimum of 1 year of skilled home health experience required.
• Current Basic Life Support (BCLS) certification is required.
• Experience with Oasis documentation is essential; please specify if you have this experience.
• Must have own transportation to manage patient visits.
• COVID-19 vaccination series plus one booster required.

Work Environment

This position operates in a home health setting, providing care to patients in their residences. The typical patient-to-therapist ratio is 1:1, with visits to up to 6 patients per day, achieving a standard of 30 productivity points. The facility makes efforts to minimize travel and driving times for therapists.
