$90 hourly

• Collaborate closely with registered nurses and therapists to develop and implement personalized care plans for each patient.
• Provide direct physical therapy services to patients in their homes, ensuring exercises and treatments are performed safely and effectively.
• Monitor and document patient progress, adjusting treatment plans as necessary to achieve optimal outcomes.
• Communicate regularly with patients and their families, offering guidance and support to enhance their understanding and engagement in the therapy process.
• Maintain accurate and timely records of patient care, ensuring compliance with healthcare regulations and agency policies.
• Participate in team meetings and case conferences, contributing insights and feedback to improve patient care and team collaboration.
• Educate patients and their families on exercises and techniques to promote continued recovery and independence between visits.

• Experience in providing physical therapy services in a home health setting.
• Ability to collaborate effectively with a multidisciplinary team, including registered nurses and therapists.
• Proven track record of developing and implementing personalized care plans for diverse patient needs.
• Strong communication skills to engage with patients and their families, fostering understanding and participation in therapy.
• Proficiency in monitoring and documenting patient progress, with attention to detail and accuracy.
• Familiarity with healthcare regulations and agency policies to ensure compliance in patient care documentation.
• Capacity to educate patients and families on exercises and techniques for continued recovery and independence between visits.
