Hospice Registered Nurse Case Manager

Gentiva Hospice in North Carolina is seeking a compassionate RN Case Manager to lead in-home hospice care, assess patients, develop plans of care, and coordinate with an interdisciplinary team. You will educate families, document care, and ensure regulatory compliance while supporting patients and loved ones through end-of-life care. This full-time RN position requires 1 year of hospice/home health experience, NC RN license, CPR, and a valid driver’s license. #J-18808-Ljbffr
