This position is for a Physical Therapist (PT) in a Skilled Nursing Facility (SNF) working day shifts from 8:00 AM – 4:30 PM, with 35 guaranteed hours per week for a 14-week contract. This is an urgent start opportunity with increased rates, focused on delivering rehabilitation services to improve patient mobility, strength, and independence.

Requirements:

• Candidates must hold an active Physical Therapist license and have graduated from an accredited Physical Therapy program (DPT or PT).
• Experience in SNF, rehab, or long-term care settings is preferred.
• Strong clinical assessment, treatment planning, and documentation skills are required.

Responsibilities include monitoring patient progress, modifying care plans, documenting sessions, and collaborating with interdisciplinary teams to ensure optimal patient outcomes and safe discharge planning.
