SummitWest Care | Mesa County, CO
Full-Time | Flexible Daytime Schedule
$48-55/hour DOE + Mileage Reimbursement

SummitWest Care is a local nonprofit home health agency seeking a compassionate Occupational Therapist (OT) to provide one-on-one care throughout Mesa County. Home health offers a different therapy experience.

Responsibilities:

• Perform occupational therapy evaluations, reassessments, recertifications, and discharges
• Develop and implement individualized treatment plans focused on safety, function, and independence
• Assess patients' ability to performs ADLs
• Conduct home safety evaluations and recommend adaptive equipment or environmental modifications
• Educate patients and caregivers on home programs, mobility strategies, and safety techniques
• Monitor patient progress and adjust treatment plans as appropriate
• Collaborate with nurses, therapists, caregivers, physicians, and family
• Supervise and support COTAs when applicable
• Complete accurate and timely clinical documentation

Qualifications:

• Current Colorado Occupational Therapist (OT) license in good standing
• Graduate of an accredited Occupational Therapy program
• Strong clinical assessment, communication, and documentation skills
• Ability to work independently in a home health environment
• Excellent time-management and organizational skills
• Valid driver's license, reliable transportation, and auto insurance

Home health experience is a bonus, but not required!
