Care Coordinator Supervisor Union Category: Non-Union Hours: Full Time Location: Duluth, MN Wage: $50,000 - 60,000 (Dependent Upon Experience) Benefit Eligibility: Yes Summary: Care Coordinators maintain responsibility coordinating and providing services for adults/children who have been diagnosed with a mental illness. This position works with a multi-disciplinary team of internal and external providers. Duties Accept assigned referrals and engage clients to initiate services. Help navigate internal and external health services. Assist in the client's integrated treatment coordination, which may include treatment planning, completing release of information(s), progress notes, and referrals to internal and external providers. Coordinate client services in areas of mental health, chemical dependency, housing, financial benefits, insurance benefits, employment, transportation, education, and other basic needs. Minimum Qualifications Two or more years of experience providing mental health services to clients. Experience navigating health, employment, housing, and/or insurance systems. Desired Qualifications Supervisory experience. Brightwater Health Advantage Flexible Scheduling Paid Supervision during licensure. Extensive training and certifications, including EMDR, TF-CBT, DBT, ECMH, and many more. Ability to work collaboratively with a team and community partners.
