The Referral Specialist executes referral management process with all referral sources and all NHC Center locations. We transition patients from the acute setting to post‑acute care, 34 Skilled Nursing Facilities (SNFs) and 19 Home Health Care agencies. We are located Pulaski TN adjacent to NHC HealthCare Pulaski. As a Referral Specialist Skilled Nursing Facility, your duties will include managing multiple referral platforms, summarizing patient referrals using medical terminology, collaborating between accepting SNF and referral source, and ensuring internal and external customers’ needs are met. You will be responsible for accurately completing referral workflows, data entry of referrals, and conversions of referrals to admissions.
