Key Responsibilities

Your daily responsibilities will include:

• Conducting comprehensive psychosocial assessments using interviews, behavioral observations, and a review of medical histories to identify emotional and social factors impacting patient recovery.
• Providing therapeutic counseling to patients and their families, helping them navigate adjustment issues, emotional distress, and the challenges of transitioning to a post-rehabilitation lifestyle.
• Collaborating with an interdisciplinary team, including physicians, nurses, and case managers, to craft robust, individualized treatment plans that address the unique psychosocial needs of each patient.
• Facilitating access to essential community resources such as support groups, vocational training, and housing assistance, ensuring that patients have the tools they need for sustained recovery.
• Participating in discharge planning efforts to ensure seamless transitions to home or other supportive environments, coordinating necessary follow-up care and resources.

Qualifications

To qualify for this position, candidates must possess:

• A Master’s degree in Social Work from an accredited institution.

Compensation and Benefits
