Remote Bilingual Behavioral Health Care Manager

Triplemoon

Triplemoon is hiring a bilingual Behavioral Health Care Manager (BHCM) to provide direct, patient-facing care to children, teens, and families within a Collaborative Care Model (CoCM). This fully remote role involves about 75% direct patient contact and delivering brief, evidence-based care designed to drive meaningful change within 6–12 months.

The BHCM will collaborate with pediatricians, psychiatrists, and a multidisciplinary team to deliver behavioral health intervention, care coordination,
