Key responsibilities include providing education and resources to ensure appropriate care progression, utilization of services, and safe patient transitions .
Qualifications include a minimum of one year experience in hospital-based Case Management or five years in Healthcare , along with a graduate of an accredited Nursing Program and Basic Life Support (BLS) certification upon hire.
This position demands leadership, critical thinking, and effective communication skills to support quality, patient-centered care and foster professional relationships.
