About the Position

Hospice Registered Nurse Profession: Nursing Specialty: Hospice RN Duration: 13 weeks Shift: Tuesday - Saturday Hours per Shift: 8:00 AM - 4:00 PM Experience: Minimum of 2 years License: Nursing License Certifications: Hospice Care Certification preferred Must-Have: Strong knowledge of pain control and symptom management Description: Utilizing the nursing process, carries out comprehensive assessment of patient/caregiver needs to provide quality care to assigned patients, in accordance with appropriate regulations and policies. Develops and communicates a realistic plan of care for each patient, ensuring that the patient/caregivers are included in the care plan process. Actively participates in interdisciplinary team meetings to develop an appropriate individualized plan of care within five days of admission and every fifteen days subsequently. Serves as Case Manager for assigned patients, actively managing case loads and visit times in accordance with the interdisciplinary team plan of care. Implements medical plan of care, nursing interventions, and procedures as necessary to ensure the safe, effective quality of care for patients. Stays up-to-date on pain control, symptom management, and high-tech skills such as IV use, maintenance, complex wound care, etc. Takes action to ensure that infection control standards are maintained throughout the plan of care. Delegates duties to LPNs within the appropriate scope of practice to ensure that the plan of care is followed and patient needs are met. Consistently reviews documentation, annotates visit notes, and acts on recommendations according to policies and procedures. Evaluates the implemented plan of care, including identified problems, care provided, and patient responses to meet patient/caregiver needs and ensure overall quality of care delivered. Maintains accurate documentation of nursing care provided to ensure the effective integration of information for ongoing quality of care. Follows agency protocol with relevant processes, including visit completion and follow-up notes. Performs patient and caregiver education to ensure informed patient decisions can be made regarding medication side effects, treatment options, and discharge instructions. Accurately completes various forms in a timely manner, ensuring they are readily available during visits to facilitate completion. Carries out supervisory visits in accordance with regulations and communicates information to relevant schedulers in a timely manner. Assesses patients' needs for support services and coordinates plans of care with patients/caregivers, providing ongoing supervision. Transmits patient incident reports to appropriate specialists in a timely manner. Coordinates safe patient discharges and/or transfers and processes all appropriate information in accordance with policies and regulations. Coordinates patient care with physicians, providers, team members, vendors, etc., in support of the recommended plan of care. Participates in peer review audits and provides constructive feedback to team members while accepting feedback professionally. Obtains prior authorization for services when needed and engages in ongoing communication regarding services rendered and any changes to the plan of care. Develops and carries out educational presentations at various facilities and community agencies. Facilitates transfers of patients to various agencies, ensuring appropriate documentation is completed efficiently. Actively participates in team meetings to maximize effective communication with team members and maintains timely documentation of meetings. Advocates for patient care issues to ensure quality and type of care is sensitive to each specific patient/family’s needs.
