You will focus on proactive measures to enhance physical health and well-being, while also taking a proactive approach to mental health. This strategic integration will minimize hospitalizations, elevate patient care standards, and increase reimbursement for your facility.Key ResponsibilitiesProvide high-quality preventative care to residents in skilled nursing facilities through on-site visits and remote workDemonstrate a patient-centered approach, building strong relationships with residents and facility staffCollaborate with our clinical team to ensure seamless alignment with facility processes and protocolsConduct thorough assessments, diagnoses, and treatment plans for patientsUtilize Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) to monitor vital signs and detect abnormalitiesDevelop and implement individualized care plans to promote physical and mental well-beingParticipate in quality improvement initiatives to minimize hospitalizations and improve patient outcomesRequirementsActive NP or PA license in the state of practice1+ year of experience in Geriatric care and Documentation reviewExcellent communication, collaboration, and teamwork skillsBenefitsHybrid Role!
