Job Summary

The Chief Regional Medical Officer (CRMO) will have overall responsibility for the medical component of Ennoble Care's home‑based primary and palliative care programs within their designated multi‑state region. The role blends clinical practice with strategic medical oversight, serving as the primary medical resource for interdisciplinary teams across the region and ensuring the provision of high quality, effective, and patient‑centered care.

Full‑time (1.0 FTE) salaried role comprising three integrated functions: independent clinical physician, House Calls medical director (mobile primary care & palliative), and Regional Leadership and Administration. Clinical practice will constitute approximately 60% of the CRMO's time, with medical director and administrative duties comprising approximately 40%, balanced to not exceed a full‑time work schedule in any given week.

Core Responsibilities

  1. Clinical Practice (Approximately 60% of time)
    • Lead by example through providing high‑quality, evidence‑based, patient‑centered care to a small panel of patients.
    • Function as an independent medical provider in the designated region (internal medicine, family medicine, geriatrics, palliative medicine).
    • Provide house calls and palliative care services for Ennoble patients in a designated geography near the CRMO’s home.
    • Deliver direct patient care including history, physical exam, differential diagnosis, treatment regimens, and ongoing chronic disease management.
    • Communicate with office staff and clinical operations across various internal platforms.
    • Ensure timely completion of clinical documentation, diagnostic review, and signing of home health orders.
  2. House Call Medical Director Services
    • Oversee provision of medical and administrative services by Company practitioners, including physicians, NPs, and PAs.
    • Provide clinical direction including regular chart reviews, develop guidelines and best practices.
    • Serve as collaborating or supervising physician for NPs/PAs as required by state regulations.
    • Act as a trusted consultant for providers requiring clinical escalation to review cases.
    • Serve as interim clinical point of accountability during provider vacancies, ensuring safety checks and urgent issue resolution.
    • Provide field training and clinical assessments for new or underperforming providers.
    • Assist in improving care delivery through protocols, operating guidelines, training, and quality assurance activities.
    • Be reasonably available for after‑hours backup call coverage for escalations or complex cases.
    • Assist in designing, testing, and improving Company technology assets, including electronic health record (EHR) software.
  3. Regional Leadership and Administration
    • Provide strategic medical leadership across the assigned multi‑state region.
    • Coordinate with local clinical leadership to ensure consistent standards of care.
    • Participate in performance improvement programs and quality initiatives.
    • Support market growth initiatives, including patient panel expansion and operational profitability targets.
    • Collaborate with regional administrators and executive leadership on business development.
    • Champion continuous improvement of core Ennoble programs such as BHI, Remote Patient Monitoring, GUIDE, and ACO.
    • Ensure compliance with Medicare/Medicaid, Stark Law, Anti‑Kickback Statute, and other applicable regulations.
    • Represent Ennoble Care at regional professional conferences, meetings, and community events.

Qualifications

Required:

  • Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree from an accredited institution.
  • Board certification by the American Board of Internal Medicine, American Board of Family Medicine, or equivalent specialty board.
  • Active, unrestricted medical license in good standing in all states within the designated region (or ability to obtain within 90 days).
  • Current DEA controlled substances registration.
  • Minimum of 5 years of clinical experience in primary care, geriatric medicine, palliative care, or hospice medicine.
  • Minimum of 3 years of medical leadership or director‑level experience.
  • Training and experience in the psychological and medical needs of homebound older adults living with chronic illness.
  • Not excluded from participation in Medicare or other federal health care programs.
  • Demonstrated ability to communicate effectively, both verbally and in writing.
  • Valid driver's license and ability to travel within the region.

Preferred:

  • Board certification in Geriatrics or Palliative Medicine.
  • Experience with value‑based care models and quality metrics.
  • Experience with home‑based primary care delivery models.
  • Prior experience in multi‑site or multi‑state healthcare leadership.
  • Fellowship training in geriatric medicine or palliative care.

Job Conditions

  • Position requires meeting deadlines and timelines required by governing and regulatory organizations.
  • Combination of office‑based, home‑visit, and remote work across multiple states.
  • Requires moderate physical activity including sitting, standing, stooping, walking, and travel by car.
  • Face‑to‑face patient visits required, which may entail moderate physical exertion.
  • Travel required by car or airplane to regional locations, seminars, conferences, and meetings.
  • Hearing, eyesight, and physical dexterity must be sufficient to perform physical assessments of patient conditions and demonstrate patient care.
  • Position may require after‑hours availability for clinical escalations and emergencies.

Equipment Operation

  • Utilization of computer, electronic health record (EHR) systems, multi‑line telephone, mobile devices, copy machine, and other office and basic medical equipment.

Compensation and Benefits

Competitive compensation package commensurate with experience, including base salary, performance bonus opportunities tied to regional KPIs (patient panel growth and profitability), and comprehensive benefits.
