* Provide *mobile primary care* to geriatric patients within *25 miles of your home*
* Manage chronic conditions (*CHF, hypertension, diabetes, etc.*) to enhance quality of life
* Collaborate with a healthcare team to ensure *consistent, high-quality* patient care

*What Were Looking For*

*Licensed NP/PA-C* in Oregon
*DEA License* (or ability to obtain within 90 days)
*BLS/CPR Certification*
Passion for *geriatric care* and chronic disease management
*Reliable transportation & home office setup*
