Empire Care Centers is expanding our clinical leadership team at Powder Springs Center for Nursing & Healing and seeking an experienced LTC nurse to step into the role of MDS Coordinator . If you have at least three years of long-term care experience, enjoy assessment and care planning, and are ready to grow into a highly valued leadership position, this is an exceptional opportunity.

This is a part-time/full-time role with a flexible schedule , and you will have the ability to participate in creating your hours while ensuring the needs of the MDS department are met.

We provide comprehensive MDS training , ongoing support, and the tools needed to succeed in this specialized clinical leadership track.

• Assessment, critical thinking, and detailed clinical review

• Care planning & interdisciplinary communication

• Clinical documentation and improving quality outcomes

• A leadership position with structure, growth, and long-term stability

As MDS Coordinator, you will play a pivotal role in driving quality measures, optimizing reimbursement, and supporting the overall clinical excellence of Powder Springs.

• Active RN license (state-specific)

• Minimum of 3 years of long-term care experience

• Strong assessment, documentation, and organizational skills

• Detail-oriented, self-motivated, and eager to learn

• Excellent communication and teamwork abilities

MDS experience is welcome but not required — full training is provided.
