Recovery Case Navigator (Bilingual a Plus)
Addiction Recovery
Hope House Treatment Center is seeking a proactive Case Manager to assist patients in their recovery journey. You will coordinate services, connect patients with community resources, and help with discharge and aftercare planning to promote long-term stability and independence.
