Full Stack Software Engineer

As a Full Stack Software Engineer at Regard, you'll own features end-to-end - from the LLM agents that synthesize patient data into clinical reasoning, to the interfaces physicians use to review and trust that output at the point of care. You'll collaborate closely with engineering, design, and product teams to bring new features from idea to production, helping solve some of healthcare's most challenging problems.

You'll shape how our LLM-powered capabilities evolve: refining prompts and agent architectures, building evaluation frameworks, developing observability and tracing tools, and closing the loop on output quality so our models improve in real-world clinical use. You'll also have the opportunity to prototype new ways physicians interact with AI-generated content at the point of care.

Our Tech Stack:

• Frontend: TypeScript, React
• Backend: Python (FastAPI, Flask, Celery), Go, PostgreSQL, Redis
• Infrastructure & DevOps: Docker, Kubernetes, AWS, GitLab CI/CD
• AI/ML: LangChain, OpenAI / Anthropic APIs, Langfuse (LLM observability), prompt engineering pipelines, RAG, LLM evaluation

Responsibilities:

• Design and iterate on LLM-powered features; including prompt pipelines, evaluation harnesses, and output quality feedback loops
• Build tooling to observe, trace, and improve model behavior in production (Langfuse, custom evals)
• Prototype new AI interaction patterns in the physician note-writing experience
• Collaborate with product and design team members to design medium to large projects balancing speed to market, user experience, business goals, and technical architecture quality
• Maintain and improve the quality of the codebase and tooling in your area of responsibility by proactive fixes and improvements, and through identifying and proposing projects to improve the overall backend codebase
• Participate in operational activities supporting including on-call, monitoring, and production support

Minimum Qualifications:

• BS in Computer Science or equivalent experience
• 3+ years of professional experience with full stack software development in one or more of the following coding languages: Python, Typescript, Javascript or Go
• Able to participate in on-call operational support for your areas of responsibility
• Able to travel up to 4 weeks a year for company co-working and/or retreat weeks
• Strong verbal and written communication skills
• Practical experience leveraging AI coding tools in day-to-day software development (e.g., Cursor, Claude Code, Codex)

Preferred Qualifications:

• Experience integrating LLMs into production applications (prompt design, eval pipelines, cost/latency tradeoffs, output validation)
• Experience with k8s, AWS services (serverless, etc), OpenAI services
• Experience in a startup and/or high growth environments
• Experience working on a mature, enterprise SaaS product
• Experience working with health technology

Hybrid Work | Location | Work Authorization

• For this role, Regard is currently only considering candidates who are authorized to work in the US without visa sponsorship, and are within the New York City metro area
• We expect our Engineers to be in the office on Tuesdays and Thursdays. We also require more frequent in-office work during the onboarding period and team onsite weeks up to once per month
• We will provide relocation assistance to anyone who does not already reside in the NYC metro area

• Additionally, hybrid employees have the flexibility to work from locations outside of their home office from up to 6 weeks per year
