Therapist - Speech Language Pathologist

Job Description

Job Summary:
The Speech Language Pathologist provides specialized speech therapy services to clients within a home health setting. The role involves direct patient care, interdisciplinary collaboration, and contributing to the development of personalized care plans to improve communication and swallowing functions.

Responsibilities:

• Assess and diagnose communication, speech, language, and swallowing disorders in clients.
• Develop and implement individualized treatment plans based on patient needs.
• Deliver speech therapy interventions to promote functional communication improvement.
• Document patient progress, updates, and treatment outcomes accurately.
• Collaborate with healthcare team members to ensure coordinated patient care.
• Educate patients and families on therapy techniques and home exercises.

Qualifications:

• Possess at least 2 years of home care speech-language pathology experience.
• Hold a valid Massachusetts Speech and Language Pathologist license.
• Demonstrate proficiency with rehabilitation equipment, including communication aids and wheelchairs.
• Have a valid driver's license and reliable vehicle for travel.
• Must have CCC (Certificate of Clinical Competence) or be a Clinical Fellow.
• Ability to work independently with minimal supervision.
