The Speech Language Pathologist provides specialized speech therapy services to clients within a home health setting. The role involves direct patient care, interdisciplinary collaboration, and contributing to the development of personalized care plans to improve communication and swallowing functions. Responsibilities:
- Assess and diagnose communication, speech, language, and swallowing disorders in clients.
- Develop and implement individualized treatment plans based on patient needs.
- Deliver speech therapy interventions to promote functional communication improvement.
- Document patient progress, updates, and treatment outcomes accurately.
- Collaborate with healthcare team members to ensure coordinated patient care.
- Educate patients and families on therapy techniques and home exercises.
- Possess at least 2 years of home care speech-language pathology experience.
- Hold a valid Massachusetts Speech and Language Pathologist license.
- Demonstrate proficiency with rehabilitation equipment, including communication aids and wheelchairs.
- Have a valid driver's license and reliable vehicle for travel.
- Must have CCC (Certificate of Clinical Competence) or be a Clinical Fellow.
- Ability to work independently with minimal supervision.
