Skilled Nursing & Long-Term Care
Uniontown Nursing and Rehab is seeking a compassionate and motivated Full-Time Occupational Therapist (OT) to join our rehabilitation team in Uniontown, Pennsylvania .

What You'll DoAs an Occupational Therapist, you will:
Evaluate residents and develop individualized treatment plans based on their needs and goals
Provide skilled occupational therapy services to improve independence with activities of daily living (ADLs)
Develop creative and effective interventions to improve strength, coordination, mobility, safety, and functional abilities
Monitor resident progress and adjust treatment plans as appropriate
Complete evaluations, progress reports, reassessments, and discharge documentation accurately and timely
Collaborate with nursing, therapy, physicians, and other members of the interdisciplinary care team
Educate residents, families, and caregivers regarding treatment plans, adaptive techniques, and safety
Participate in care planning and interdisciplinary team meetings
Maintain compliance with facility, state, and federal standards
Current Occupational Therapist (OT) license in Pennsylvania or eligibility for licensure
Graduate of an accredited Occupational Therapy program
Strong communication and interpersonal skills
Compassionate and resident-centered approach to care
Ability to work collaboratively within an interdisciplinary team
SNF/LTC experience is a plus, but not required
