Responsibilities
- Provide direct and indirect skilled nursing care to residents under the supervision of an RN, following established policies and procedures.
- Collect data on residents’ physical and psychosocial status and communicate findings to the RN and Interdisciplinary Team.
- Contribute to development of an age-appropriate, culturally relevant care plan reflecting nursing diagnoses and interventions; involve resident and family in the care planning process.
- Recommend revisions to the plan of care based on outcome evaluation and coordinate referrals to ensure comprehensive care.
- Maintain current nursing knowledge and competency; participate in mentoring or orientation of staff; demonstrate leadership within the professional practice setting.
- Maintain safety and infection control standards; handle situations with potential exposure to blood or body fluids in accordance with policy.

Requirements
- Licensure: Active New York LPN license
- Certifications: BLS certification (American Heart Association or RQI)
- Experience: At least 1 year of LPN experience in a long-term care facility
- Education: Graduation from an approved practical nursing program (LPN)
- Ability to provide nursing care to adult and geriatric residents; ability to work within a skilled nursing environment under RN direction
