Clinical: RN Clinical Supervisor- Per Diem

Per Diem RN Supervisor - Home Care Pay & Compensation Sup/Post Hospital: $60 Reassessment: $75 Initial Admission: $90 weekday; $125 weekend Visiting Client Territory Schenectady and surrounding areas. Schedule Evenings and weekends. Visits planned with required presence of the patient's aide. Office Address Schenectady, NY Mileage Reimbursement Provided at the current federal rate. Responsibilities Performs on-site supervisory and teaching visits to evaluate caregivers. Provides clinical orientation to caregivers on assigned home care cases. Participates in the training of new nurses and in‑services for field staff. Performs initial nursing assessments. Counsels patient/caregiver in meeting nursing and related needs. Assigns and coordinates services provided. Assists physician in developing a plan of treatment/care and makes necessary revisions based upon patient’s condition/needs. Prepares clinical progress notes ensuring any changes in the patient’s condition is reported. Monitors clinical records for complete and accurate documentation. Qualifications Registered Nurse (RN). CPR Certified. 1 year of Home Care experience required. 1 year of supervisory or management experience required. Valid NYS Driver’s License, insurance, registration, and reliable car.
