Physical Therapist (PT) Per Diem Job Summary: We are seeking a dedicated and compassionate Physical Therapist (PT) to join our team at Terraces of Lake Worth Care Center, a leading provider of Rehabilitation services in Lake Worth Beach, Florida. As a Per Diem PT, you will have the opportunity to work with a diverse group of patients and participate in providing high-quality care in a supportive and dynamic environment. Responsibilities: Conduct physical therapy evaluations and develop individualized treatment plans for patients in our sub-acute and skilled nursing care settings Implement and modify treatment plans to address patient needs and goals, ensuring effective progress and recovery Collaborate with interdisciplinary teams, including nursing, occupational therapy, and speech therapy, to ensure comprehensive care Provide education and support to patients, families, and caregivers on physical therapy techniques and strategies Maintain accurate and timely records of patient progress and treatment plans Participate in quality improvement initiatives and contribute to the development of best practices in physical therapy About the Role: As a Per Diem PT at Terraces of Lake Worth Care Center, you will be an integral part of our dedicated rehabilitation team.
