Care Coordinator

Lifeline Vascular Care

About the Organization Lifeline Vascular Care works with premier physician practices to develop and manage outpatient centers that provide focused and coordinated vascular access procedures to patients receiving hemodialysis treatment for end-stage renal disease (ESRD). We currently manage more than 40 physician‑owned vascular access centers where we provide physician practices with financial and outcomes reporting, staffing and training, facility operations, regulatory compliance, and insurance billing services. The Lifeline network of managed vascular access centers is accredited by the Joint Commission. Through our relationships with nephrologists, vascular surgeons, interventional radiologists and interventional cardiologists, we create and fortify environments in which these crucial services are delivered both seamlessly and efficiently. By delivering care in this manner, patients receive proactive care, early intervention and are able to maintain a quality of life that is all too often compromised by disease. By exceeding industry clinical standards for quality care and having the best patient outcomes in the nation, we achieve a high level of clinical credibility that assures patients and practices that our centers are best in class. General Purpose of the Job The Care Coordinator is responsible for managing and streamlining the continuum of vascular access care for patients requiring dialysis. This role serves as a key liaison between patients, nephrologists, surgeons, dialysis facilities, and other care team members. The Care Coordinator ensures timely scheduling, follow‑up, and communication for all stages of vascular access, from initial catheter placement through long‑term fistula or graft maintenance while promoting patient education and positive health outcomes. Essential Duties and Responsibilities Patient Navigation & Scheduling Serve as the central point of contact for patients and care teams throughout the AV access process. Coordinate referrals and schedule appointments for vein mapping after initial catheter placement. Arrange pre‑surgical clearance and scheduling for AV fistula or graft creation. Schedule surgical procedures and confirm all pre‑operative requirements are met. Post‑Surgical & Ongoing Care Coordination Monitor fistula maturation progress through follow‑up communication and imaging when needed. Coordinate evaluation and clearance for cannulation readiness. Facilitate catheter removal once the fistula/graft is ready for use. Maintain ongoing surveillance of long‑term fistula or graft health, liaising with dialysis teams, nephrologists, surgeons, and endovascular specialists (EVASC). Documentation & Communication Maintain detailed and accurate patient records in the EHR. Communicate proactively with patients, families, and providers regarding timelines, procedures and expectations. Track milestones in the AV access workflow and ensure timely interventions at each stage. Quality & Compliance Ensure adherence to clinical protocols, regulatory standards, and HIPAA requirements. Identify workflow improvements and participate in quality initiatives related to vascular access management. Minimum Qualifications Education, licenses, certifications and experience required to fulfill the essential duties, including computer skills as required. Nephrology experience preferred. Experience in teaching adult learners strongly preferred with demonstrated ability to communicate vascular access/medical information in lay terms as necessary. Scheduling flexibility. Current driver’s license in state of residence and insurable driving record required. Language Skills Fluent in the written and verbal skills necessary to perform successfully the essential functions, duties and responsibilities of the position. Ability to understand clinical/medical vocabulary written or spoken in English required. Vision Requirements Vision adequate to perform the essential duties and responsibilities of the position including repetitive use of computer monitor. Physical Demands Ability to lift a maximum of 20 pounds unassisted and able to stand sit stoop walk stretch and reach with moderate range of body motions. Work Environment Job required tasks normally do not involve exposure to blood body fluids or tissues, but may require performing unplanned Level I tasks. The normal work routine involves no exposure to blood body fluids or tissues; however, exposure or potential exposure may be required as a condition of employment and appropriate personal protection measures are required in those instances. Position Requirements High school diploma required. One year experience in a healthcare or medical billing office. Intermediate to advanced computer skills and proficiency in MS Word, Excel and Outlook. Ability to multi‑task and manage time to complete various duties with busy incoming phone lines and hold times with insurance companies. Provide exceptional customer service. Ability to deal with confidential information and issues involving discretion and judgement. Works with others in a cooperative and supportive manner.
