Hybrid Case Manager for Aging & Disability
Pierce County
Pierce County’s Aging and Disability Resources Division seeks an experienced case manager to support older and disabled adults in the Medicaid program. You will assess needs, authorize services, oversee care plans, and monitor progress using a laptop in clients’ homes, with a hybrid work arrangement after the initial in-office period. You will join a team dedicated to helping seniors remain safe and independent at home, with a focus on timely responses, diligent documentation, and strong #J-18808-Ljbffr Pierce County
